Name Change
Warning! If you have already applied for a passport using the Post Office or the U.S. Government, we will not be able to assist you in expediting your process. Please contact 1-877-487-2778 or click here for assistant on the status of your application.
You need to send the following:
STEP 1 – FILL OUT A DS-5504 FORM
You can pick up this application (DS-5504 form) from your local Post Office or Court House or you can download it here.
STEP 2 - YOUR PASSPORT
The passport MUST be valid and signed.
**If your current passport is more than a year old from the date of issue, you must apply for a Passport Renewal (DS-82 Form). Please click here for our Passport Renewal link. Please also note that the fees are going to be different.
STEP 3 - PROOF OF NAME CHANGE
You have to submit an original marriage certificate, divorce decree or name change court decree showing the change of name. The name change document must bear the official seal of the issuing authority. Photocopies or notarized copies will not be accepted.
**All documents will be returned to you once your passport is done.
STEP 4 - PASSPORT PHOTOS
We will need two 2” x 2” passport photos.
Passport photos are available at most FedEx/Kinkos or Walgreens/CVS locations.
Please note: acceptable passport photos are:
• Photographs must be 2 x 2 inches in size, taken within the past 6 months.
• The image size from the bottom of the chin to the top of the head should be between 1 inch and 1 3/8 inches.
• The photographs must be in color.
• They must be full face, front view with a plain white or off-white background.
• Photographs should be taken without a hat or headgear that obscures the hair or hairline.
• Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless required for medical reasons.
• Home made Digital photographs are not acceptable.

STEP 5 - PROOF of DEPARTURE
This is required ONLY if you wish to expedite your new passport processing by using Express or Rush processing, you must provide an itinerary or airline ticket showing your departure date departing 5 days from the day of your application.
If you do not have an itinerary, we can obtain one for you. Please contact customer service at support@tltravel.net.
STEP 6 – LETTER OF AUTHORIZATION
Please print and sign TWO Letters of Authorization, which will allow TL Travel to expedite your passport on your behalf.

**Applicant’s signature MUST match the signature of their passport, and HAS to be original. No copies or faxed Letter of Authorization will be accepted.
STEP 7 – ORDER FORM
Please fill out our order form before returning your documents to TL Travel.

STEP 8 – FEES
The Government fee for a name change is $60 (only if your passport is less than a year old). If your passport is over a year old, you must do a Passport Renewal.
