Lost Passport
Warning! If you have already applied for a passport using the Post Office or the U.S. Government, we will not be able to assist you in expediting your process. Please contact 1-877-487-2778 or click here for assistant on the status of your application.
If you have lost your passport, or it has been stolen, the U.S. Department of State requires that you apply for a brand new passport. To do so, please click the NEW PASSPORT button to the right.
In addition, you are required to fill out and sign the following DS-64 and DS-11 forms.
Please read the instructions on the New Passport page carefully and make sure you have all of the required documents before you proceed with your Passport application.
*Please fill out and sign the form in front of the Acceptance Agent at the Post Office or Courthouse.
**Failing to provide any of the required documents WILL result in the delay of your application process and cause complications for you and your trip.
